Our Participant Interview Forms
Our pharmacists and staff use our Participant Interview Forms to conduct medication therapy management and tailored community referral. This set of three copyrighted forms includes the most recent version of our initial interview, 6-month, and 12-month interview forms (electronic copies). They are made available for a $75 one-time fee that allows you the right to reproduce and utilize them. The forms are a question-by-question guide for a comprehensive assessment and have been written, reworked, and refined over our 16+ years of operation.
Our Geriatric Formulary
This copyrighted geriatric formulary lists the medications that are safer, more effective, and more affordable for older adults. Generic and brand names are listed, preferred medicines are highlighted, and prices are shown by categories. It fits on one page (front and back), and the drugs are categorized by disease. The formulary was created and is updated every six months by a panel of healthcare professionals (pharmacists, physicians, nurse practitioners, and others), many of whom are trained in geriatric medicine. A yearly "subscription" includes the most recent version plus one update, which will be emailed to you when it is released.
*In the coming year, Senior PharmAssist plans to offer consulting to other organizations and communities that wish to begin similar programs.
